If it is necessary to cancel an appointment, a 24 hour notice is required. You will be charged a cancellation fee for notice less than 24 hours.
Follow Up Appointments
Payment is required at the time of service. We accept Visa, Mastercard, Discover, American Express and personal checks. Third party checks are not accepted.
We will recommend a time frame for a follow up appointment. Please make this appointment with our office staff before you leave. If that is not possible, contact our office (630) 357 4608 at your earliest convenience to assure a time that is in accordance with your schedule.
You may contact us by phone for questions that cannot wait until your next appointment. Please call (630) 357 4608. If the office is closed, the answering service will take your call and contact your provider. Please be prepared to inform the person answering the call the nature of your concern.
The use of email to communicate with your provider is not meant for serious or urgent issues. If you believe that you have an urgent issue that needs to be addressed please call the office. Responses to email may take up to 72 hours, and we do not routinely check email. Email may not be as secure as other forms of communication, raising concerns about patient privacy. Mistakes in addresses, hacking, shared email accounts or use of your employer's email all raise the possibility of a breach of confidentiality. Please keep this information in mind when using email to communicate with us.
Refills are normally provided at the time of your appointment. Please check your medications and request your refills at that time.
If you need a refill between appointments, please contact your preferred pharmacy and ask that they contact our office to request a refill. Specify the name of the medication, the dosage, how frequently you take the medication and the number of pills prescribed for 30 days. Allow 48 hours to complete a refill from the time of your request. Refills are not provided on weekends or holidays.
Please note--refills for stimulants cannot be phoned in or requested by your pharmacy for refill. Prescriptions for medications such as Adderall, Concerta, Ritalin, or Vyvanse can be sent electronically to your pharmacy. For these medications, please call our office directly or contact our staff at email@example.com. Specify the name of the medication, the dosage, how frequently you take the medication and the number of pills prescribed for 30 days and the pharmacy location. Allow 48 hours to complete a refill from the time of your request. Refills are not provided on weekends or holidays.
