Emergencies

     The use of email to communicate with your provider is not meant for serious or urgent issues. If you believe that you have an urgent issue that needs to be addressed please call the office. Responses to email may take up to 72 hours, and we do not routinely check email. Email may not be as secure as other forms of communication, raising concerns about patient privacy. Mistakes in addresses, hacking, shared email accounts or use of your employer's email all raise the possibility of a breach of confidentiality. Please keep this information in mind when using email to communicate with us.

     Refills are normally provided at the time of your appointment. Please check your medications and request your refills at that time.
     If you need a refill between appointments, please contact your preferred pharmacy and ask that they contact our office to request a refill. Specify the name of the medication, the dosage, how frequently you take the medication and the number of pills prescribed for 30 days.  Allow 48 hours to complete a refill from the time of your request. Refills are not provided on weekends or holidays.

     Please note--refills for stimulants cannot be phoned in or requested by your pharmacy for refill. Prescriptions for medications such as Adderall, Concerta, Ritalin, or Vyvanse can be sent electronically to your pharmacy. For these medications, please call our office directly or contact our staff at drcapristaff@gmail.com.  Specify the name of the medication, the dosage, how frequently you take the medication and the number of pills prescribed for 30 days and the pharmacy location. Allow 48 hours to complete a refill from the time of your request. Refills are not provided on weekends or holidays.

     Payment is required at the time of service. We accept Visa, Mastercard, Discover, American Express and personal checks. Third party checks are not accepted.

Payment Policy

If there is a risk of immediate harm to yourself or someone else, call 911 or go to your nearest Emergency Department.

     We will recommend a time frame for a follow up appointment. Please make this appointment with our office staff before you leave. If that is not possible, contact our office (630) 357 4608 at your earliest convenience to assure a time that is in accordance with your schedule.

     If it is necessary to cancel an appointment, a 24 hour notice is required. You will be charged a cancellation fee for notice less than 24 hours.
