Please bring a list of all current medications, including dosage and frequency, and the aforementioned forms.

     To schedule your first appointment, please contact our office at (630) 357 4608. Expect to set aside one hour for your initial evaluation. Please arrive 10 to 15 minutes before your scheduled appointment, to allow for completion of necessary forms to begin your care. Follow up appointments are scheduled for 20 to 60 minutes, depending on the complexity of your illness, at the discretion of your provider.

Please be advised that, at this time, we are operating fully remotely to maintain safety for our patients and staff.  It is extremely important for everyone to maintain social distancing.  
We recommend patients only leave their homes for essential needs, such as food, medications, caring for loved ones who require your assistance or critical work.
Although we prefer face to face interactions, we are currently only providing tele-psychiatry services.
Our phones are answered by our staff working remotely, so please leave a voicemail if the staff do not answer immediately. Use our office number for communication, and for routine matters our staff email: drcapristaff@gmail.com.

How do I Schedule My Appointments and How long will They Take?

     We are "out of network" providers for insurance carriers. As benefits vary according to insurance plans, please contact the number on your insurance card to inquire about your level of benefits. We do not accept Medicaid, and we have opted out with Medicare. Our staff will submit claims to your insurer, and you will be reimbursed directly by the insurer. Payment is required at the time of service. We accept Visa, Mastercard, Discover, American Express, and personal checks. Third party checks are not accepted.
